POLST. MOLST. POST. MOST.

The first letter could be P for provider or physician, or M for medical. The OLST stands for Orders for Life Sustaining Treatment. Some states add the first letter of the state name, too.

There are variations on the acronym, but the meaning is the same - a standard medical order outlining a patient’s choices on life-sustaining treatment. A POLST form is typically completed for patients who meet the “surprise question” criteria:

a “no” answer to the query “Would you be surprised if this patient died in the next year? (timeline can vary)
